Care Coordinator

Play a pivotal role in organising and coordinating care delivery for our clients. You will manage rotas, supervise support staff, liaise with families and professionals, and ensure every Service User receives care that truly reflects their needs and wishes.

Location: Henola Care Ltd —field-based within our service area. You may be required to work from otherlocations with appropriate notice.

Working Hours: 5 days over a 7-dayperiod, as agreed with the Registered Manager.

 

Role Overview

The Care Coordinator is theoperational heart of Henola Care. You will organise and oversee care deliveryfor our clients, manage staff rotas, supervise Support Workers and act as thekey point of contact between clients, families and the wider team. You willensure every Service User receives care that is person-centred, timely andconsistent, and that staff are well-supported in their roles.

 

Key Responsibilities
Care Coordination

•       Organise and co-ordinate caredelivery in accordance with best practice, policy, legislative requirements andrelevant regulations

•       Ensure care is delivered toService Users at the times and for the duration specified in their individualagreements and contract standards

•       Ensure Service Users are at theheart of care delivery — their wishes and preferences must always enhance theirwellbeing

•       Ensure each staff member isintroduced to the Service User before or at the beginning of their first visit

Rota & Workforce Management

•       Develop and maintain the duty rotato meet Service User needs, ensuring adequate staff with the correct skill mix

•       Manage annual leave requests toensure fair allocation and sufficient staffing at all times

•       Ensure staff allocated to eachService User have the relevant skills, knowledge and resources for thatperson's needs

•       Manage travel delays and staffshortages, minimising the impact on Service Users

Leadership & Supervision

•       Provide day-to-day leadership andsupervision to Support Workers

•       Induct and orientate new employeesto their job role and the service

•       Organise and conduct regularsupervisions and appraisals for care staff

•       Dispense medication in accordancewith Henola Care policies and procedures

Record Keeping & Compliance

•       Maintain all Care Plans and carerecords in accordance with company policy

•       Audit care records regularly toensure compliance with company policy and regulatory standards

•       Promote and safeguard the welfareof all individuals supported by the service

Communication

•       Build rapport with Service Usersby establishing genuine personal connections and showing interest in theirlives

•       Communicate any changes, concernsor problems to family members and relevant professionals as required

•       Maintain an excellent telephonemanner when dealing with staff and Service Users, including under pressure

•       Work in cooperation withmultidisciplinary team members to maximise opportunities for Service Users
